Private Testing Request Form

To request a private testing session, please fill out the form below. It is highly recommended you make your request at least two weeks in advance.  Private testing sessions are only available Tuesday to Friday, excluding holidays.

Note: All rescheduled confirmed appointments are subject to a rebooking fee: $25 for the first rebooking, and full payment for any subsequent rebookings.
